EXAM
CBCT stands for cone-beam computed tomography. CBCT systems used by dental professionals rotate around you, capturing data using a cone-shaped x-ray,These data are used to reconstruct a three-dimensional (3D) image of the following regions of the patient’s anatomy: dental (teeth); oral and maxillofacial region (mouth, jaw, and neck); The radiation doses are lower than the medical CT.

Root canal treatment (endodontics) is a dental procedure used to treat infection at the centre of a tooth.
The infection at the centre of a tooth (the root canal) is caused by bacteria that live in the mouth and invade the tooth.

Root canal treatment is an often straightforward procedure to relieve dental pain and save your teeth. Patients typically need a root canal when there is inflammation or infection in the roots of a tooth. During root canal treatment, dentist will carefully removes the pulp inside the tooth, cleans, disinfects and shapes the root canals, and places a filling to seal the space.
You should avoid biting on hard foods and advice to have the crown to protect the tooth.
Wisdom teeth, or third molars, are the last permanent teeth to erupt in the mouth. These teeth usually appear between the ages of 17 and 25. Some people never develop wisdom teeth.
Many people develop impacted wisdom teeth — teeth that don’t have enough space to erupt into the mouth or develop normally. Impacted wisdom teeth may erupt only partially or not at all.
If a wisdom tooth is impacted, resulting in pain, infection or other dental problems, We will advice to pull it out for long term oral health.
Assessment:
When considering wisdom teeth extractions specifically, one of the most critical questions is how close the Inferior Dental (ID) nerve runs to the roots of your wisdom tooth. This only applies to the lower teeth.
Just because it looks close doesn’t actually mean it is; the OPG is a 2D picture of a 3D structure. If the wisdom teeth is closed to ID nerve, CBCT is indicated to assess more precisely.
Procedure:
The surgery will be performed under local anaesthesia, sedation anaesthesia or general anasthesia , depending on the expected complexity of the wisdom tooth extraction and your comfort level.
An incision will make in the gum tissue to expose the tooth and bone. Piezosurgery is used to remove the bone which is proved to be a useful and safe technique. The tooth will divide into section and removed. Suture will place to close the wound.

Laser Dentistry
The tooth is divided into two parts: the crown and the root. However, the crown that is exposed outside the gum is called the “clinical crown”. The crown lengthening surgery uses traditinoal surgery or waterlaser to remove the gum or even the bone covering the teeth, which aim to expose a greater amount of tooth structure (“clinical crown’)around a tooth or group of teeth.
Traditional surgery is an open flap surgery,it removes some of the gum tissue, bone, or both to expose more of the tooth that needs to be covered. In some clinical situation, the dentist can use Waterlaser for the surgery without open the flap.
Common reasons for crown lengthening include:
- The damaged tooth broke at the gum line.
- The gumline is too far along the tooth for restorative treatment.
- There is not enough tooth remaining.
- A previous crown or filling has fallen off, exposing decay underneath.
- Gummy smile
Also known as gingival hyperpigmentation, or gingival pigmentation, those brown or black skin splotches that sometimes occur naturally on gum tissue are normally caused by excess melanin, natural pigments also found in our hair and even eyes. Individuals most commonly affected are those with dark skin. Smoking and long-term use of particular medications can also cause this hyperpigmentation on the gum tissue, but more often than not it’s just the result of genetics. In any case, patient concerns are normally just aesthetic ones.
At the Perfect Smile Esthetic Centre, we use the latest technology Waterlase Laser to safely and successfully treat these dark gum splotches in a process we call gum-depigmentation. The procedure normally takes less than an hour.
Teeth whitening (dental bleaching)
Vital bleaching was first reported in 1989. However, the technique actually originated over 20 years eariler
In dental bleaching, hydrogen peroxide diffuses through the organic matrix of the enamel and dentin. Because the radicals have unpaired electrons, they can react with most unsaturated high molecular weight and break it into smaller molecular weight. If the tooth is healthy and intact, dental bleaching is very safe procedure.
The effectiveness of the treatment depends on the shade of the tooth, not every treatment has ideal result. The dentist will use value-shade guide(It have 16 value grades from bright to dark.) to match the shade before the treatment.
The effect of the dental bleaching will sustain around 8 to 10 months , depend on the patient diet and habit.
The dentist will recommend the patient who have the teeth whitening, should be avoided the drink/food that will get stained ( 2 hours for home bleaching, 24 hours of laser bleaching), otherwise, the teeth will get stained again. For the longevity of the treatment, the patient should reduce the coffee/ soft drink. wine.
The patient is also recommended to have good oral hygiene habit.

Dental Implant
A dental implant is a piece of titanium or other materials that looks like a screw. It is put into the jaw where the missing tooth’s roots were. Over time, bone grows around the implant, which helps to hold it in place. That phenomenon is called osseointegration.
Dental implant already has more than 40 years and the sucess rate is more than 95%. After the osseointegration, the crown can be installed in the implant. Dental implant not only function good and also esthetically pleasant. But it need to have enough bone and soft tissue around, and have enough space around the adjacent teeth.
Implant is a good option to replace the missing tooth. The other option is dental bridge and it need to prepare the two adjacent teeth in order to support the missing teeth.
Dental implant can used to replace missing tooth and also support the denture or fixed full mouth bridge.
All-on-4 dental procedure , invented by Portuguse dentist Paulo Malo, refers to’all’ teethbeing supported ‘on four’ dental implants, a prosthodontics procedure[for total rehabilitation of the edentulous (toothless) patient, or for patients with badly broken down teeth, decayed teeth, or compromised teeth due to gum disease.
Advantage:
- IIt minimizes the bone augmentation procedure by placing two implants vertically in the anterior region and two oblique implants in the posterior region. By placing two oblique implants, it can avoid the maxillary sinus and inferior alveolar nerve.
- Immediate loading with minimal trauma. The patient can have soft diet after surgery.
Disadvantage:
- For the plan of the final prosthesis, some of the alveolar bone may need to removed.
- Need to have good oral hygiene for the long term periodontal health. Otherwise, one of the implants failed will affected the prosthesis.
The lab will merge the patient photo , STL file of the intra-oral scan and Dicom data of CBCT scan for the implant surgyery. In addition, the lab will also prepare the virtual smile design in the photo for the assessment.

It’s snug enough to stay in your mouth, yet easy to take in and out. The Invisalign aligner allows you to eat any foods because they won’t get stuck in metal braces. More importantly, you can easily brush and floss your teeth by temporarily removing the aligner.
There are many different brand of aligner in the market. But Invisalign brand have a lot of patency and research inside the aligner. It is the best and no 1 in the market.

Smile Esthetic
DSD is a technical tool which is used to design and modify the smile of patients digitally and help them to visualize it before hand by creating and presenting a digital mockup of their new smile design before the treatment physically starts. We can then do the physical mock up in the patient without touching the tooth.
After the motivitonal mock up, we can proceed the treatment for the patient smile.

Single Crown in esthetic area
It is a common clinical situation, in our clinic, Dr.Ryan Tse will used special photographic technique, i.e. cross polarised filter and elab card, for the shade matching.
Microabrasion
The superficial brown stain in the tooth surface, can be removed by Opalustre( Ultradent Products)( photo 1)，which is 6.6% hydrochloric acid slurry contains silicon carbide microparticles. This combination provides chemical stain removal along with gentle mechanical abrasion. OpalCups latch-type bristle polishing cups are used with the Opalustre slurry for micropolishing the newly treated enamel surface.
ICON infiltation (DMG Germany)
The enamel white spots are caused by air and water entrepments in the tooth which have a lower refractive index than intact tooth structure, which leads to unesthetic discolorations. Icon(Photo 5) balances out this difference and the appearance blends in with the healthy enamel, and is helpful for treating a variety of white spot lesion types.
The tooth will be discolored after root canal treatment or by traumatic extravastion of blood products.This internal discoloration can be treated by the application of an oxidant paste, a mixture of sodium perborate and 3% to 30% hydrogen peroxide placed in the pulp chamber.
Procedure :
The endodontic material is removed and dental material is applied to isolate the chamber as a barrier. The bleaching gel is placed inside the chamber and then sealed with dental material.
The bleaching gel is replaced every week till the desired result is reached. The tooth chamber is replaced with composite resin.
The result may relapse after 3-4 years, retreatment may be indicated after.
